Yeah, none of my e-addresses worked: maybe I typed in a wrong address when I upgraded? I have to go back and fill-out the form now. I hope that gets some results!
I actually used this form at 3AM this morning to report the problem, but just sent a second notice. I also emailed the following, with a screen-copy of the charge to my bank account:
