It's simple but tricky. Once the song is in the bear share library (meaning you've downloaded it successfully) it is automatically in the bear share file folder. Minimize your bear share screen. Go to itunes and click "File" then scroll down to "Add Folder to Library" click "Music" in the left hand column and you should see a Bear Share folder or icon. Right click on this icon chose "select" from a small list that pops up and you should then see a list of the songs you've got on your bear share library. Click "select folder" (down at the bottom of this screen) and a notice from iTunes will pop up telling you already have those songs on iTunes (if you previously added them successfully but forgot) or that it needs to convert them to mp3 which iTunes will then do automatically. The song(s) should now be in your music library on iTunes. It took me HOURS to figure this out but it really works and I tried everything else.

I just tried it out with BearShare 5.1b (which was connecting to 20 UP's.) I discovered if I simply drag & dropped a fresh audio file into my BearShare download folder, within a short time iTunes would open & copy it to its own music library. I think it depends on your settings in both Bearshare & iTunes.

That audio file I tried out was not previously in the iTunes music library. (It was a recently downloaded rare concert song via torrents; the band's re-union concert for a special appeal, no recording ever officially released. I would have bought it otherwise since I'm a hardened fan of theirs.)
